Outreach mission

Our initiative is dedicated to addressing minority health disparities and promoting health equity within marginalized communities in Wake County and its surrounding areas. Through targeted outreach efforts, we aim to tackle various health challenges faced by these communities, including:

  • Disease Awareness and Prevention
  • Obesity Reduction among minorities within our communities
  • Substance Abuse Prevention
  • Mental Health Promotion

Outreach Strategies

Our Outreach Initiatives are dedicated to bolstering low-income communities through enhanced health awareness and educational resources. Our outreach projects are carefully designed to employ evidence-based or promising practice models, ensuring we effectively improve community health and showcase tangible outcomes and sustainability. At the heart of our mission lies a commitment to raising awareness about our health education programs and services, with a particular focus on preventing chronic diseases and promoting healthier lifestyle choices.

In Our Outreach Program we are dedicated to

Encourage Conversation: Having discussions with a wide range of partnerships allows us to find opportunities that align with our mission. This leads to deeper relationships within our communities.

Participate in Local Events: We participate in community events that involve local businesses, which are great places to set up a booth and interact within the community. It also raises awareness of our organization and allows us to share our content to help the community.

Be a Partner for Your Community: A key to engaging local communities is understanding the issues that affect them the most. The community must view us as a partner that will be there for the long term with their concerns in mind.

Host Events, Trainings and Seminars: We host health fairs that includes nurses and educators to administer blood pressure and glucose checks, distribute preventive health education materials, provide consultation and maintenance measures for Cardiovascular Disease, Diabetes, Obesity and more. We provide monthly virtual and in-person education seminars and health awareness discussions.

Our Community Health Assessments (CHA) is a continuous process that helps us identify the strengths and barriers that impact the health of our communities. The information gathered drives our community-based programs and shapes our collaborative partnerships to create regional goals to improve health and create a culture of wellness.
