Community Health Outreach

Evidence-based outreach addressing disease awareness, obesity reduction, substance use, and mental health promotion in marginalized communities of Wake County and surrounding areas.

About This Program

Reaching Communities Where They Are

Our outreach initiatives employ evidence-based or promising practice models, ensuring we effectively improve community health and showcase tangible outcomes. We tackle challenges including disease awareness, obesity reduction, substance use, and mental health promotion.

We position ourselves as long-term community partners building relationships through trust, showing up at local events, hosting education seminars, and bringing health screenings directly to the people who need them most.
